# Context — I pressed Run, the command worked, and Claude stayed silent until I typed again

Claude Code runs in a permission mode we keep strict. Some commands it will not run on its own: merges, production deploys, anything that destroys state. It hands them back as text, and the operator runs them by hand. One week's audit counted 37 commands run by hand and 22 refusals turned into hand-offs.

A mod is a Claude Code plugin that can watch events and draw in the terminal. We wrote one, handed-commands, that collects the commands Claude hands back and shows them in a pane with a Run button. Press Run, the command executes, and Claude should see the output and continue.

The pane, the button and the execution all worked in `claude plugin test`. The test kit has no `session.append`, so the one step that mattered, getting the output back to Claude, was never exercised. Eight tests were green on a feature that did not do its job.

The mods API changes between Claude Code builds. This account is for the 2.1.291 test kit.
